Case Management

Case management integrates all care of clients. Case Managers help guide the course of treatment and ensure that all involved providers are working together towards common goals. Multi-systemic care includes community, neighborhood, and spiritual sources of support that are accessed by Case Managers who have knowledge of local resources. This approach to wraparound care empowers families to create a support system that is responsive to their needs. It is culturally appropriate and meets the needs of a diverse client population, integrating natural support systems with professional clinical care.

Youth entering therapy are frequently involved in more than one system of care. They may have an assigned Child Protective Services (CPS) worker as well as a Probation Officer. They may be in special classes in school, or attending an alternative school. They may have been placed in a group home or foster care at a young age, or may still be living in an out-of-home placement. Quite often, young people referred for behavioral health services have more than one clinical diagnosis with which they must contend. Effective case management ensures a streamlined approach to integrated care.

Providence Case Management provides treatment programming responsive to the client's level of need.

• Intensive Case Management is offered by highly trained clinicians who supervise the treatment of clients in acute care, therapeutic group homes, residential treatment, and partial hospital programs, or returning home from a residential program. The case manager maintains a close working relationship with the family, provider facilities and outside agencies to determine strengths-based treatment goals and facilitate coordinated treatment planning.
• Case Management is also provided for clients who do not require out-of-home placement, yet their level of functioning in the home or community remains markedly impaired. At this level of care clients may require treatment coordination due to multiple agency involvement. The case manager works as part of the clinical team to monitor treatment planning and develop strengths-based treatment goals. The case manager also assists with identifying natural support systems and enlisting their help with the family.
• Case Management for clients requiring moderate to low intensity of service is provided by the therapist working with the youth and family. Typically, these clients are not involved with multiple agencies and require only home- or school-based counseling or behavior management. Treatment is supervised by the therapist and agency.

Case Management significantly reduces the need for out-of-home placements of our clients and helps maintain the integrity of our client families. The Case Manager is part of a multidisciplinary Clinical Team overseen by a Board Certified Psychiatrist, all of whom work in concert to meet the needs of the client.
